Participation in the National Health Insurance program (NHIP) is mandatory. INGOs are required to apply for group insurance at the National Health Insurance Administration (NHIA) upon receiving approval to establish an office, civil association, or foundation. All local employees and foreign employees holding an Alien Resident Certificate (ARC) must be enrolled in the NHIP. Additionally, responsible persons of nonprofit organizations must be enrolled in the NHIP as employees. Employees participating in the NHIP enjoy the same benefits but are classified by employment status and pay insurance premiums accordingly. Submit applications to the relevant regional division of the NHIA.
1. INGO NHIP group insurance application and NHI enrollment

(2) Written application: Send the following documents (items a. to f.) by mail to the relevant regional division of the NHIA. (See address list below.)
- A. Completed Group Insurance Applicant Establishment Declaration Form.
- B. One photocopy of the front and back of the National Identification Card of the responsible person. (If the responsible person is not an R.O.C. citizen, provide one photocopy of the individual’s ARC or passport).
- C. One photocopy of the business license or related registration, one photocopy of the approval letter issued by the competent authority of the sector, and photocopies of any other relevant documents.
- D. Completed National Health Insurance Application Form.
- E. For foreign employees, one photocopy of the employee’s ARC and the work permit letter from the Ministry of Labor.
- F. Completed NHI Card Claim Application Form (Attach one photograph of the employee and photocopies of the identification documents).
